Keynote Session and Q&A: Hospice at Home Toolkit: resources to optimise end of life care

The Hospice at Home toolkit is a free-to-use online resource for hospice at home providers, members of the public, health and social care commissioners, and wider palliative care workers. The toolkit is based on evidence from a national evaluation of Hospice at Home services and has been co-designed with the above groups. This presentation will focus on what the toolkit offers for service providers; particularly a service assessment and guided information on what other services have done to optimise their delivery. We will also discuss the findings of our evaluation, showcasing how the toolkit has benefitted services which have piloted it.

This talk will involve a brief but deep dive into what documents we all should have in preparation for end-of-life. Jane will also talk about the role of the end-of-life doula and conclude with some thoughts on funerals – why they are important and how to get it right. She hopes there will be time for Q&As so come prepared!

 

The value of touch, to support end of life care

For carers, nurses and practitioners, touch is integral to our work. In this session, we will be exploring the life of touch and how the response to it heightens, especially as one comes closer to the end of life. We will also consider how, through mindful touch and plant oils, we can facilitate warmth and comfort during this time.  

There is also the essential element of self-care. I will share some simple methods to ensure that you too are nurtured and nourished, enabling you to do the work you love while having enough energy for yourself and loved ones at home.  This session will conclude with a guided hand massage, giving some loving attention to our hard-working hands.
